Center For Sight is committed to providing you with the highest quality eye care and best customer service. Just call any of our three locations Monday-Friday, 8:30 a.m.-5:00 p.m. to schedule an appointment. Our answering service is available for after-hours calls.
Please be ready to provide the following for your appointment:
- Social Security Number
- Driver’s License/Picture ID
- Insurance Card(s)
- Insurance Authorization (if required by your carrier)
- List of Medications
- Glasses and/or Contact Lenses
If you are a new patient or if more than a year has passed since your last visit, please arrive at least 20 minutes before your appointment so that we can update your information, or complete the online patient forms and bring them with you to your appointment.

LASIK Financing
Some insurance companies may pay for all or part of vision correction procedures. You can check with your plan administrator, or we will be happy to check for you. Many people have medical flex plans that they can use to save up to 50% on their procedure by using pre-tax dollars. Your employer’s human resources department can tell you whether you have this benefit. Vision correction also may be tax deductible as a medical expense. (Check with your financial advisor.)
If you do not have insurance coverage, we do offer affordable payment plans. We also accept cash, personal checks, Visa, MasterCard, and Discover.

Center For Sight participates in numerous insurance plans; however, it is the responsibility of the patient to review his/her insurance coverage and to know if a referral, pre-certification and/or a second opinion is necessary. Co-payment is due at the time of service.

Insurance Claims
Center for Sight will submit to your insurance carrier, provided we have all the necessary information including a copy of your insurance cards (primary and secondary). However, it is important to remember that professional services are rendered and charged to the patient, not the insurance company. Even though an insurance claim is filed, you will receive a statement each month showing the status of your account. We are happy to file your claims, but we cannot accept responsibility for collecting your insurance claim or for negotiating a settlement on a disputed claim. If your insurance carrier has not paid the balance within 60 days from the time we filed the claims, you may be required to pay the balance.
Payment Arrangements
Please contact our billing office at 850.476.9236 to discuss your bill. Bills that receive no response after 3 billing cycles may be forwarded to collections.
Center For Sight will file your insurance claims for you if you have a Medical Diagnosis or if your insurance company is one that we are contracted with for “wellness” or a “vision rider.”
Center For Sight does not file insurance claims for a determination of refraction. This is a test to determine a prescription for glasses. Payment for this test is due at the time of service.
