Appointments

Center for Sight is dedicated to providing you with out-of-sight care and customer service. Make your appointment now by calling any of our three locations. Appointments scheduled Monday through Friday from 8 a.m. to 5 p.m. at all locations, and at our Pensacola office on Saturday from 9 a.m. to 1 p.m.

 Please be prepared to provide the following when you arrive for your appointment:
• SSN
• Driver’s License/Picture ID
• Insurance Cards
• Insurance Authorization (if required by your carrier)
• List of Medications
• Glasses and/or Contact Lenses

 If you are a new patient or if more than a year has passed since your last visit, please arrive at least 20 minutes before your appointment so that we can update your information, OR simply complete the online patient forms and bring them with you to your appointment.
