Center for Sight participates in numerous insurance plans; however, it is the responsibility of the patient to review his/her insurance coverage and to know if a referral, pre-certification and/or a second opinion is necessary. Co-payment is due at the time of service.

Center for Sight will submit a claim to your insurance carrier, provided we have all the necessary information including a copy of your insurance cards (primary and secondary). However, it is important to remember that professional services are rendered and charged to the patient, not the insurance company. Even though an insurance claim is filed, you will receive a statement each month showing the status of your account. We are happy to file your claims, but we cannot accept responsibility for collecting your insurance claim or for negotiating a settlement on a disputed claim. If your insurance carrier has not paid the balance within 60 days from the time we filed the claims, you may be required to pay the balance.
