Payment Arrangements

Please contact our billing office at 850.476.9236 to discuss your bill. Bills that receive no response after 3 billing cycles may be forwarded to a collections agency.  Center for Sight will file your insurance claims for you if you have a Medical Diagnosis or if your insurance company is one that we are contracted with for “wellness” or a “vision rider.”

Center for Sight does not file insurance claims for a determination of refraction. This is a test to determine a prescription for glasses. Payment for this test is due at the time of service.
